Chile and Costa Rica were two of the Latin American countries that configured their health systems earlier at the beginning of the 20th century. The current demographic and epidemiological indicators that both countries have been the result of a long process of organized social responses carried out by multiple actors, generating simultaneous and differentiated institutional construction processes. In order to carry out a historical approximation of this process, the objective of this article is to compare the initial configuration of the health systems of Chile and Costa Rica during the first half of the 20th century. For this, it is proposed to use as categories of analysis the organizational and institutional responses from charity, hygiene, philanthropy and the social issue.
